   Notes on the Electoral Roll
If your 16th birthday falls in the next 12 months, you may complete the form and become eligible to be entered on the roll on your birthday. If you have been on the electoral roll before and have been prevented from 'habitual attendance' at church during the last 6 months through illness or another essential reason, please tick the box and add to the end of the form 'I would have habitually attended, but was prevented from doing so because....' and add your reason. Churches in Communion with the Church of England are other Anglican Churches and certain foreign churches. Membership of the electoral roll is open to members in good standing of a Church not in communion with the Church of England which subscribes to a doctrine of the Holy Trinity.
